FINE PRINT & TERMS OF MEMBERSHIP

  • The Med Spa Membership has a minimum six (6) month commitment. There is no freeze or pause with the Med Spa Membership.

  • After the minimum commitment is fulfilled, a cancellation request can be submitted in writing a minimum of 30-days prior to your bill date

  • Payment is taken when you sign up for the membership. You are billed $250 to the credit card on file on the first of each month thereafter.

  • If payments payments are declined or missed for more than two (2) months (consecutive or nonconsecutive), the membership may be canceled, and you will not be able to re-enroll within a 12-month period.

  • Use it or lose it! Med Spa Membership member treatments must be redeemed by the member each month. Member treatments may not be shared, or transferred to another patient who purchased this program, or for whom the program was purchased for.

  • Members are allowed one missed treatment credit of $250 per calendar year. The treatment credit can be used towards product purchases only. Due to the limited availability of our memberships, no additional monthly treatments are permitted, only product purchases.

  • If members miss two (2) months (consecutive or nonconsecutive) of treatments, the membership may be canceled, and you will not be able to re-enroll within a 12-month period.

  • With limited availability for membership appointments, members are encouraged to book in advance. While we do our best to accommodate schedule requests, members may need to schedule with the next available clinician to be seen and avoid forfeiting a monthly benefit.

  • Members who decide to have advanced treatments such as Radiofrequency (RF) Microneedling Series of 3, Opus Plasma Treatments, or surgery with Dr. Zamora can verbally request to have their memberships put on an Advanced Hold. Billing will resume three (3) months after the Advanced Hold is activated unless otherwise directed by the member’s clinician or by Dr. Zamora. Member perks and discounts will be valid on Advanced Hold.

  • If the six (6) month minimum commitment is not fulfilled, a cancellation fee will be assessed to the card on file, as penalty for not meeting the minimum commitment required of the membership.

  • Membership may change or be discontinued any given time.
